What is a Personal Assessment Questionnaire (PAQ)?

The PAQ (Personal Assessment Questionnaire) is similar to the DBQ (Disability Benefits Questionnaire). However, while a DBQ can only be completed and signed by a medical provider, the PAQ is a self-assessment that allows veterans to document their current service connected conditions.

How Does the PAQ Help?

The DBQ is a form required by the VA, and it must be filled out by a licensed doctor. However, to make the process smoother for both clients and doctors, VetVantage asks veterans to complete a PAQ before their appointment

  1. Self-Assessment: Clients answer some of the same questions from the DBQ in advance.
  2. Doctor Review: The provider can quickly review the client's answers and make necessary adjustments
  3. Faster Appointments: If the responses are accurate, the doctor can proceed efficiently, ensuring the client’s condition is reflected correctly

By completing the PAQ, veterans help streamline their evaluations, making it easier for doctors to fill out the official DBQ accurately and efficiently
